Filing a Claim

Ask the caller for their policy number.  Check to see if their policy number starts with ESP, if so, you will need to fill out a Notice of Service Event Form (ESP).  When you click the link, it may error out, but there should be an option at the top of your browser to open in Adobe Reader. If not, you should have a copy of the form in your email as a temporary measure.

If it is an ESP claim, follow the ESP Claims process to file the claim.

If your caller just wants to ask about the process of making a claim and is from a university, ask if he purchased a CCRA membership card through Safeware. If he answers yes, then directly transfer your caller to Jenna Burkhart via her number (614) 781-2592. (Do not use her extension for transfers.)

For all other claim requests such as the following:

  1. Check status of claim (for non CCRA members)
  2. File a non ESP claim
  3. Update information on existing claim (for non CCRA members)

Transfer them to the Claims department using the HUD Transfer Processes.
