Exercise of Voting and Other Rights

Certain voting and other rights may apply to an investment option that you have chosen for your account.   Those rights may be exercised by the plan sponsor or other named fiduciary for the plan who retains the right to exercise on behalf of the plan voting, tender and similar rights with respect to the designated investment alternatives in the plan.

Administrative Expenses

Your employer has elected to pay the cost for administrative services provided to the plan. These services are necessary for the operation of your plan and can include recordkeeping, custodial, legal, and accounting services.

 

Advisor Fees
Expenses for investment advisory services provided to the plan may be charged to your account. Your employer has directed that advisor fees be charged pro rata based on the amount of each participant’s account balance. Charges will depend on the value of your account. Based on our records, the annual fee and per participant fee is estimated in the table below. This estimate is subject to change as it is based on varying factors such as the number of participants in the plan and current balances. Actual fees may vary from this estimate or may not be deducted from plan accounts in some circumstances. The actual amount charged to your account will appear on your participant statement under the Your Expenses section.

 

Individual Expenses

The expenses listed below for services provided to you may be charged to your account.  If any of these expenses are charged to you, they will be deducted from check proceeds or from account assets. 401(k) Notice of Eligible Automatic Contribution Arrangement (EACA)

IMPORTANT: Carefully read and consider the following information before you decide whether to start making elective deferrals or decide to continue or change the amount of your employee 401(k) contributions.

GENERAL INFORMATION

Your retirement plan allows you to elect to have contributions (called deferrals) made on your behalf to the plan in lieu of receiving the amount as cash compensation. This type of plan is commonly referred to as a 401(k) plan. In order to make saving for retirement as easy as possible, your plan includes an Eligible Automatic Contribution Arrangement (EACA) so that you may be automatically enrolled when you become eligible. If you do not enroll in the plan by the date requested by your plan administrator, the percentage of compensation listed below will be withheld from each paycheck and contributed to the plan as a deferral. In addition, if you do not have investment elections on file, these dollars will be invested in the plan’s default investment described below. If you have questions regarding this automatic enrollment feature please contact the plan administrator listed in this notice.

AUTOMATIC DEFERRAL CONTRIBUTION INFORMATION

You have the right to elect to have any amount permitted under the plan withheld from your paycheck, including no amount at all. If you do not make an election, the percentage of compensation listed below will be withheld from your paycheck each pay period until you complete a salary reduction agreement and submit it by following the procedures established by your plan administrator indicating that you do not wish to have amounts withheld or you wish to increase or decrease the amount of your deferral. You may complete a salary reduction agreement to make or change your election by (specify a date at least 30 days from the delivery date of this notice). This timeframe to make or modify your salary deferral election is in addition to any other election modification periods specified below. Automatic Compensation Reduction and Increases During your initial period of participation in the EACA you may be automatically enrolled at the deferral rate indicated below. Following your initial period, your initial deferral rate will increase according to the table below unless you complete a salary reduction agreement specifying a different deferral rate.

Initial Amount of Automatic Elective Deferral If you are automatically enrolled in the plan, your plan administrator will automatically withhold and contribute to the plan 3% of your compensation.
Automatic Deferral Increases Your plan administrator will automatically increase the amount of your deferrals by 1% of compensation per paycheck up to a maximum rate of 10%.
Timing of Increase The automatic increase of your deferrals will occur on the first day of each plan year.

EXAMPLE 1: Your plan administrator automatically enrolls participants at a rate of 4% of their compensation into the plan. The automatic enrollment will apply to you if you satisfy the plan’s eligibility requirements and do not enroll in the plan during the designated time period. You will automatically be enrolled in the plan and 4% of your compensation will be contributed to the plan rather than being paid to you as compensation.

EXAMPLE 2: Your plan administrator automatically enrolls participants who do not make a deferral election and defers at a rate of 4% of their compensation into the plan. You satisfy the plan’s eligibility requirements and you make a specific election of 0%. Because you made a specific election regarding your deferrals, you will not be automatically enrolled in the plan and none of your compensation will be contributed to the plan.

Making A Deferral Election

You may change the amount you are deferring into the plan —or stop making deferrals altogether—at the times indicated below by notifying your Plan Administrator of your desire to change your deferral rate using the method approved by your plan administrator.

Stopping Your Deferral Per payroll period. Please contact your plan administrator for information around the timing of payroll updates.
Beginning Deferral Again After You Stopped Making Deferrals Per payroll period. Please contact your plan administrator for information around the timing of payroll updates.

DISTRIBUTIONS

Withdrawals Under an EACA

You will be permitted to withdraw (without penalty) that portion of your deferrals that has been automatically contributed to the plan under the EACA.

Your election to withdraw must be made within 90 days following the payroll date that your first deferral was deducted from your paycheck and automatically contributed to the plan.

If you choose to withdraw your deferrals, your withdrawal will also consist of any earnings attributable to those deferrals. Any matching contributions made by your employer that are related to those deferrals will be forfeited.

If you do not make investment elections, amounts contributed under the automatic enrollment feature of the plan will be invested in the default investment(s) listed below. Your plan administrator will provide you with information on the plan’s procedure(s) for making investment elections, including other investment alternatives available under the plan.

Enrollment Steps

 

 

Employee Signup for the First time

  1. Navigate to the Employee Website at: https://my.vanguardplan.com/vanguard/account/login
    .
  2. Click the Get Started button.
    .
  3. Verify your Social Security Number, your Date of Birth, and your Zip Code and click Next.
    .
  4. Set your username and password.
    (Password must have three of the following four)
    – Uppercase letters
    – Lowercase letters
    – At least 1 number
    – At least 1 special character
    Click Next when done.
    .
  5. Enter in an email address to use for this account – a personal email address is recommended. Type in your email address and click Next.
    .
  6. Check that email address for an email from accoutservices and enter the provided verification code into the website. Click Verify when done.
    .
  7. Once verified, click Next.
    .
  8. If you have a smart phone, you can select to add Text to secure your account. If you do not have a smart phone, you can select to add Voice to secure your account.
    .
  9. Enter your phone number and click Send Code to Verify.
    .
  10. Enter in the code you received by Text or Voice and click Verify.
    .
  11. If you do not wish to edit your number and you do not wish to add another phone number, then click Next
    .
  12. Verify that the information looks correct. If you so chose, click Trust This Device.  – Then click Complete, then Continue.

  13. When you are ready, click on Start Enrollment.

  15. Once done and you’re satisfied, put a checkmark next to the acknowledgement and click on Continue Enrollment.

  20. Set your deduction amount and toggle if you want your rate to increase by 1% annually or not..
    (if you are declining, set your percent amount to be 0 and the Increase Savings Rate to No.)

  21. Click Continue when done. You may need to click OK on the verification pop-up … make sure you stop and verify that the amounts you set are what you want before clicking OK.

    (at this point, the instructions will continue for those who elected an amount to be deducted that’s not 0.)

  22. Click on Continue Enrollment.

  23. Click on Continue Enrollment for section 4.

  24. Depending on your preference, you can either Make Changes or Leave as is. … Click on your preference.

    Make Changes selection – (From here, you can make your investment selections into the optional provided Investments.

    If you need guidance on which investments to choose, you can contact Vanguard by phone at 866-794-2145. You can also consult with a personal financial advisor.

    Once done, and ready to move on, you can click on Save.
    .
    You’ll have to click on Continue Enrollment to return to step 4, and click on Leave as is to continue.

  25. Verify your investment selection(s) and click on Continue Enrollment.

  26. Click on Continue Enrollment for section 5.
    .
  27. Click Designate Your Beneficiaries.
    .
  28. From here, use the tabs What do I need? and Learn More to see what you need for this page. Return to the Designation tab when ready and then click on Designate Your Beneficiaries.

  29. Select your marital status and then click Continue.
    .
  30. Select your Beneficiary Type from the drop down, then fill out the fields that pop up.
    .
    (The allocation percentage is the share of your plan assets each beneficiary will receive at the time of your death. When you allocate a percentage to each beneficiary, you must use whole percentages, and the allocation must equal 100% between all your designated primary beneficiaries, and separately for your designated contingent beneficiaries.)

  31. Review the page, and once you’re satisfied, click Continue.
    .
  32. If you need to Add a Primary Beneficiary or Add a Contingent Beneficiary, click on either, and follow steps 25 – 31 as listed above. Otherwise, click Review.
    .
  33. The system gives you a final chance to go back and add Beneficiaries. If you want to do so, click Return to Add and go back to step 32 above. Otherwise, click Continue with Review.
    .
  34. Under the Authorize & Submit section, click on the blue Disclosure to review it; this will open the disclosure in a new tab on your web browser. Delete that tab when you are done reading it.

  35. Once you are ready click on the checkbox next to “I am applying an e-signature to this document … “

  36. If you wish to modify your Beneficiaries, click on Modify and go back to step 32 above.
    .
  37. From here, you can either Submit by Fax/Mail or Submit Electronically. Click on your selection.

    If you selected Submit by Fax/Mail, follow the instructions on your screen and click on the option to continue.
    .
  38. If you selected Submit Electronically, then that will be done and you’ll be on the “has been completed” page.
    .
  39. If you want to, you can click on View your Beneficiary Designation to view the information.
    .
    B. If you decide to make changes, click the Update button and then Continue, then move to step 29 above. Otherwise, click on Return to Enrollment.
    .
  40. From here, if you wish to Designate Your Beneficiaries, click that option and go up to step 29 B above.
    .
  41. If you do not wish to make any further changes with your Beneficiaries, click the checkbox next to “I have completed my beneficiary designation or will complete it at a later time.” and click on Continue Enrollment.

  42. Click on Continue Enrollment for section 6.
    .
  43. Review the page. You can click on print if you wish to print this page or click Cancel to cancel section 6. Otherwise, click Continue Enrollment.

  44. If you wish to Review any of the steps, note that you can click Cancel, to return back to step 44, or click Logout to end the process.

    If you Review 1. Enrollment Presentation, then click to do so and go up to step 14.
    If you Review 2. Plan Highlights, then after reviewing the page, you can click Cancel to return back to step 44, or click Logout to end the process.
    If you Review 3. Set your savings rate, then click to do so. If you wish to make a change, select the rate you wish to adjust, click Continue, and go up to step 20, or click Cancel to return back to step 44.
    If you Review 4. Select your investments, then click to do so and go up to step 24.
    If you Review 5. Complete your beneficiary designation, then click to do so and go up to step 40.
    If you Review 6. Confirmation, then after reviewing the page, you can click Cancel to return back to step 44, or click Logout to end the process.

Enrollment FAQs

I know that the personal information I entered at the registration step is correct. Why am I receiving an error message?
Check with Ken Becker to make sure the social security number, date of birth, and zip code that he submitted to the system is correct and if he needs to take extra steps to get this resolved.

I am not sure what to select for my personal investments. Is there someone I can talk to for advice?
You can contact Vanguard by phone at 866-794-2145 – You can also consult with a personal financial advisor to discuss the options that best support your financial goals.

I do not know my username and password. Where can I get this information?
If you are registering for the first time, you will need to select, “Get Started” on the my.vanguardplan.com homepage to create your username and password.

What should I use as my password?
Create a secure and unique password that is 8-25 characters long. It must contain at least 3 of the following:

  • Upper case letter
  • Lower case letter
  • Number
  • Symbol

Can I enroll into the retirement plan by phone?

Do I have to complete the registration and enrollment at the same time?
No, the website tracks your progress if you need to complete the enrollment over multiple sittings. Online enrollment allows you to enroll at the same time of day that is most convenient.
